Conference Center.

ELIGIBILITY
FOR MEMBERSHIP
To be eligible
for membership in the Fourth Degree, candidates must be:
(1) Over 18 years of age; (2) A Third Degree member in good
standing in his Council, and who was initiated into the First Degree prior to October 18, 2009; and (3) A practical Catholic
in union with the Holy See, and (4) a US Citizen, or willing to Pledge Allegiance to USA.
Fees: The basic fee for candidates is $70.00. A special fee for priests and members of religious orders is $35.00.
Full payment should accompany each candidate’s application and reservation form.
Each candidate receives one (1) banquet ticket to the Fourth Degree Banquet.
Additional banquet tickets are available at a cost of $45.00 per person. The
fee for the Ladies' Activities is $25.00. All checks must be made payable to:
Master, Fourth Degree Knights of Columbus. All
applications, banquet reservations, outstanding fees, and Ladies' Activity reservations must be received by the District Secretary
on or before March 15, 2010. There will be no refunds or changes after this date. Please forward all applications and reservations to SK Ernest E. Harley, Secretary
to the Master, 14721 Harvest Lane, Silver Spring,
MD 20905-5642. To ensure that all applications are accurately completed, the candidate’s application must be received,
voted on, and approved by the Assembly before it is submitted to the Secretary to the Master.
Hotel: To make reservations Call
1-800-228-9290, ask for the Knight of Columbus Room block, refer to the Group code kgckgca. Mention which night you would like
your stay (April 17, 2010 –April 18, 2010). A credit card will be necessary
to reserve your room and they will charge the first night including all taxes to your card.
Be sure to examine the confirmation very carefully to avoid any confusion when you arrive. If you don’t receive a confirmation, call the Marriott to follow up.

DRESS
The official dress for candidates for the Fourth Degree Exemplification is black tie, plain white tuxedo shirt with plain collar, black cummerbund, black shoes and socks, and black tuxedo. Religious may wear their clerical attire, and members of the armed forces or full
time police and firefighting officers may wear their respective dress uniform. Members
shall wear the official regalia, that is: the official dress with the social baldric
LADIES’
ENTERTAINMENT
For Ladies, there will be a social and entertainment program beginning at 11:30 AM. This will include
fun, prizes, and refreshments. The price is $25.00. Afternoon wear is suggested. Dress for the reception and banquet
will be evening wear. Please check with your Assembly for dressing room accommodations
for ladies needing to change for the Banquet.
Reservations
for ladies attending the ladies’ social and entertainment activities, and banquet must be made on or before March 15,
2010.

BANQUET RESERVATIONS
A Banquet ticket for the
Candidate is included with the Initiation Fee. All other Banquet tickets are
$45.00. Checks should be made payable to Master,
Knights of Columbus Fourth Degree.
DEADLINES
To ensure that all applications are completed accurately, the candidate’s application must be received and approved by the Assembly before submission to the Secretary
of the Master. It is important, therefore, that candidates and/or proposers
contact the Faithful Navigator or membership director for any Assembly specific information.
Applications, Banquet Reservations for members and guests, and Ladies
Activity Reservations should be made on or before March 15, 2010.
The deadline for
receipt of applications by the Secretary is March 12, 2010.
All balances must be paid in full on or before March 15, 2010. There will be no refunds for cancellation after this date.

COMPLETING YOUR APPLICATION
The following sections of
the Membership Document (Form 4) must be completed. Please follow these instructions:
Section 1: Enter your name,
address, city, state and Zip Code, telephone number, date of birth, Marital Status, First Degree date, and the Council in
which you joined the Knights of Columbus.
Section 2: Enter Country
of citizenship and whether by birth or naturalization. If you are a naturalized
citizen, complete the portion regarding final papers.
Section 3: Does not pertain to Candidates.
Section 4: Complete your
Parish name and sign the application. Have your proposer enter his membership
number and sign the application as well.
Section 5: Reserved for
Assembly approval that includes signatures of the Faithful Navigator and Faithful Comptroller.
Section 6: Enter your membership
number listed on your Third Degree card. If you do not know your membership number,
contact your Financial Secretary.
Have your Council Financial
Secretary complete the certification part of the application. Complete the insert
of this brochure and return it with your check for deposits and/or fees with your Form 4 to your proposer or Council Fourth
Degree Liaison. Either will submit all of your paper work to the Assembly’s
Faithful Navigator.

Program Ad Book Information:
As with past Exemplification’s, the District will publish
a program book for the new Sir Knights and banquet attendees. We ask all Assemblies
and Councils in the Archdiocese of Washington District to take an ad in this program book.
The costs of the program ads are:

The deadline for all ads will be March 20, 2010. Send the ad in now and avoid the rush. After the program goes
